We offer aesthetic treatments (Botox®, dermal fillers, microneedling), regenerative therapies (peptide therapy, PRP, hormone optimization), IV therapy (signature drips, Myers' Cocktail, NAD+, custom blends, B12/MIC+ shots, and add-on enhancements), and comprehensive wellness consulting including GLP-1 medical weight loss, screenings, immunizations, and preventative care.
Félix Vida Wellness is a cash-pay clinic and is not contracted with insurance providers. This allows us to offer more personalized, flexible care without the limitations of insurance-based models. We accept HSA and FSA funds, as well as all major credit cards, cash, and digital payments (Venmo, Zelle, Apple Pay). Superbills are available upon request for potential out-of-network reimbursement. Patients are responsible for submitting these directly to their insurance provider.
We require at least 48 hours' notice to cancel or reschedule. Cancellations with 24–48 hours' notice incur a $50 fee. Cancellations within 24 hours are charged 50% of the service cost, and same-day cancellations (within 4 hours) are charged 100%. No-shows are charged the full service cost. Deposits are non-refundable for late cancellations and no-shows. If you have a genuine emergency, please contact us as soon as possible — we review those situations individually. Services that have been rendered are non-refundable. If you pre-pay for a service or package that hasn't been used, you may request a full refund within 7 calendar days of purchase. Partially used multi-session packages may be refunded on a prorated basis within 90 days of purchase, calculated using the current single-session price for sessions already received. Deposits are non-refundable for late cancellations and no-shows. All refund requests must be submitted in writing to support@felixvidawellness.com.
We recommend arriving 10 minutes early, especially for your first visit. If you're more than 15 minutes late, your appointment may need to be shortened or rescheduled to ensure we can provide you with adequate treatment time and not impact other patients' schedules. Abbreviated treatments due to late arrival are not eligible for refunds.
You can easily reschedule through our online patient portal at our booking page, or contact us by phone or email. Just be sure to give us at least 48 hours' notice to avoid cancellation fees. We're flexible and will do our best to find a time that works for your schedule.
All treatments are performed by Kaitlyn Félix, MS, MSN, RN, a board-certified registered nurse with advanced training in aesthetic medicine. Kaitlyn practices under standing orders and the medical direction of Dr. Christopher Ray, MD, who provides clinical oversight and physician-level supervision for all services we offer. This ensures every treatment meets the highest standards of safety and clinical care.
A Good Faith Exam (GFE) is a clinical assessment performed before certain aesthetic procedures — including Botox®, dermal fillers, microneedling, PRP, and IV therapy. It allows your provider to evaluate your health history, assess any contraindications, and ensure the planned treatment is appropriate and safe for you. Under the Arizona Board of Nursing's guidance, a GFE is required for Level II and Level III aesthetic procedures. It may be conducted in person during your first appointment or via telehealth prior to your visit. Your GFE is renewed annually or any time your health history changes significantly.
For new patients, yes — a thorough consultation and Good Faith Exam are part of your first visit. This allows us to understand your health history, goals, and contraindications, and create a personalized treatment plan. Follow-up visits typically proceed directly to treatment based on your established plan. We also offer a complimentary 15-minute phone or video consultation so you can ask questions before committing to an in-person visit.
Yes. Certain conditions and medications may disqualify you from specific treatments or require medical clearance. Common contraindications include: active skin infections or open wounds at the treatment site, blood clotting disorders, certain autoimmune conditions, and specific medications (such as blood thinners for injectable treatments). We will review your complete health history before every service. Please disclose all medications, supplements (especially fish oil, aspirin, vitamin E, and other blood thinners), allergies, and relevant medical history. When in doubt, consult your primary care provider before booking.
Most of our services are not recommended during pregnancy or while breastfeeding. Neuromodulators (Botox®), dermal fillers, and most IV therapy formulations have not been studied in pregnant or breastfeeding populations and are generally contraindicated. If you are pregnant, trying to conceive, or currently breastfeeding, please inform us at the time of booking. We will work with you and, if appropriate, recommend you consult your OB/GYN before proceeding with any treatment.
GLP-1 medical weight loss uses medications such as semaglutide and tirzepatide, prescribed under physician oversight, to support sustainable weight management. These medications work by regulating appetite and blood sugar. We offer both FDA-approved brand-name versions (Ozempic®, Wegovy®, Mounjaro®, Zepbound®) and compounded formulations sourced from licensed 503B outsourcing pharmacies. Compounded formulations are not FDA-approved but are prepared in accordance with federal and state pharmacy compounding regulations. GLP-1 medications may be appropriate for adults with a BMI of 30 or higher, or a BMI of 27+ with at least one weight-related condition such as type 2 diabetes, high blood pressure, or high cholesterol. Candidacy is determined during a medical consultation with our provider. GLP-1 medications are not recommended during pregnancy or breastfeeding. Visit our Pricing page for program details.
Treatment times vary by service. Botox®/neuromodulator appointments are typically 15–30 minutes. Dermal filler appointments run 30–60 minutes. IV drip sessions (including signature drips and custom blends) are 45–60 minutes. B12 and MIC+ shots take just 5–10 minutes. Microneedling (with or without PRP) treatments are usually 60–90 minutes. GLP-1 weight loss consultations are 30–60 minutes, with follow-up check-ins at 15–30 minutes. First visits include a consultation and GFE and may take longer. Your provider will give you a time estimate when you book.

Our services are designed for adults and are generally available to patients who are 18 years of age or older. In very limited circumstances, certain services may be available to patients under 18 with a parent or legal guardian physically present and with valid written consent — and only at the provider's clinical discretion. We do not offer Botox®, dermal fillers, or IV therapy to minors.
Félix Vida Wellness is a HIPAA-covered entity and we take your privacy seriously. Your protected health information (PHI) is never sold, shared for advertising purposes, or disclosed without your authorization except as required by law. We use HIPAA-compliant platforms for booking and records (OptiMantra), encrypt all data in transit, restrict record access to authorized staff only, and provide you with full rights to your records including access, amendment, and restrictions. Please contact us immediately if you experience any unexpected symptoms after your treatment. You can reach us by calling or texting 602.456.0324 or emailing support@felixvidawellness.com. For mild reactions (temporary swelling, bruising, or redness), this is often normal and will resolve. For severe or concerning symptoms — difficulty breathing, severe swelling, signs of infection, or vision changes — call 911 immediately. We are available for follow-up consultations at no additional charge for concerns directly related to your treatment.
Wear comfortable clothing that allows easy access to the treatment area. For instance, for IV therapy, short sleeves or a loose top works best. For facial treatments, arrive without heavy makeup if possible. Your comfort is our priority.
Absolutely. All treatments are evidence-based and performed by Kaitlyn Félix, RN under the medical direction of Dr. Christopher Ray, MD. We use only FDA-approved products, follow strict clinical protocols, conduct a Good Faith Exam before every procedure, and take the time to assess each patient individually. We carefully evaluate each patient individually to determine treatment appropriateness.

Cancellations
